.PDF Form Instructions:

You must have Adobe Reader Version 8 or higher installed on your computer to access the form.  This program is available to download for free on the Adobe website. 

Download the form to your computer or disk and open with Acrobat Reader (do not use Mac Preview). Once all of the information has been entered, click on the ‘submit by email’ button at the end of the form. Select either “Desktop Email Application” or “Internet Email” in the Select Email Client window:

• Via desktop application (such as Outlook or Mac Mail): This will automatically attach the completed form to an email, with a populated to field and subject line.
• Via internet email (such as Juno, Yahoo or Gmail): save the filled form to your computer and attach it to an email message addressed  to zolnikp@caryinstitute.org. You must enter “REU Application Submission” in the subject line.

If you are a Mac user and cannot use the submit button, save the file ( file> save as> OR file>save a copy) in Acrobat Reader and attach to your email message as outlined above.

Note: YOU MUST USE ACROBAT READER. DO NOT OPEN, SAVE OR FILL-OUT THE FORM USING MAC PREVIEW OR YOUR BROWSER'S ACROBAT PLUG-IN.
